Blended Case Management provides concrete service to adults age 18
and older diagnosed with a serious mental illness living in the
townships of Huntington and Smithtown. The goal of the program
is to assist these individuals to attain and maintain their
maximum level of independent functioning coordinate resources to
avoid inpatient hospitalization. Teams consisting if Intensive
Case Managers and Supportive Case Managers visit in individuals
on a regularly scheduled basis in their homes. They assess
client needs, help them to establish goal plan and link them to
community resources. Case Managers routinely advocate on behalf
of their clients and assist them in obtaining service such as:

• Financial Entitlements
• Housing
• Outpatient Mental Health Care
• Vocational Placement and Training
